Anime Expo Artist Alley 2017 Form

This form is to be filled out by the Table Buyer only. Read the 2017 Artist Alley Packet to ensure your form is submitted correctly.

Seller's Permit: If you are conducting sales, you must specify the ID number of a valid California Seller's Permit under your own name. You must also keep the original permit at your table for the duration of the convention as required by California State Law.
If you do not have your seller's permit yet, you will be able to login to your profile and enter it in at a later date upon completion of this form. Your profile will also allow you to make any updates or edits to your additional guests at a later date.
Age Requirements: Participants must be 18 years old at the start of AX 2017. Participants who are 16 or 17 years old must have their parent or guardian with an Artist Alley badge at Anime Expo at all times and must complete and submit a Parent Consent Form. The consent form will be sent out at a later date. Participants who are 15 years old or younger are not allowed to participate in Anime Expo Artist Alley.
